CLINICAL SIGNIFICANCE
A1-Microglobulin is a low molecular weight glycoprotein, stable at pH alterations. Its function hasn’t been adequately clarified. Increased levels in urine under normal circumstances of glomerular filtration are indicative of reduced tubular re-absorption, a typical trait of chronic interstitial nephropathy or acute / chronic tubular damage. A1M can be used as indicator of interstitial tubular nephropathy or for its exclusion, with high level of certainty. In renal failure the plasma levels of this protein increase from an early stage. It can also rule out all forms of primary and secondary Fanconi syndrome, heavy metal poisoning, nephrotoxic medicine side effects and renal rejection reactions after transplants.
